837 COMPLETE MAGNESIUM BIORESORBABLE SCAFFOLD ABSORPTION AT 1-YEAR OF INTRACORONARY IMAGING FOLLOW UP

Abstract Background the development of bioresorbable scaffolds (BRSs) has been conceived as revolution in interventional cardiology for prospective improvements treatment coronary artery disease. However, metallic drug eluting stent (DES) demonstrated greater safety and efficacy than vascular scaffold (BVS) Absorb dedicated trials culminating commercial withdrawal device 2017. Nevertheless, progressive absorption restoration physiological characteristics attracted attention, so currently research on BRSs is progress. Case presentation a 58-year-old woman was admitted our department after an episode typical angina at rest without significant troponin elevation. The electrocardiogram showed T wave inversion lateral leads. Hypokinesia distal septum anterior wall mildly reduced left ventricular ejection fraction (LVEF 50%) were observed echocardiography. Her cardiovascular risk factors included obesity, arterial hypertension, dyslipidaemia, family history ischemic heart disease former smoking. past medical remarkable chronic kidney (CKD EPI eGFR 50 ml/min) previous connective tissue Coronary angiography highlighted critical stenosis middle segment descending (LAD). Optical coherence tomography (OCT) imaging recanalized organized throumbus calcific plaques. After adequate predilatation sizing by OCT, 3×25 mm Magnesium-based BVS (Magmaris, Biotronik AG, Buelach, Switzerland) implanted. OCT confirmed good expansion apposition postdilatation. Patient discharged dual antiplatelet therapy (DAPT) 12 months, anti-remodeling cardiac lipid-lowering therapy. One year later, patient underwent elective control showing complete reabsorption Magmaris with positive remodelling luminal enlargement vessel. asymptomatic throughout performance status. Conclusion present case illustrates example imaging-based revascularization optimal intracoronary result 1 follow up agreement last available evidence. European Society Cardiology/European Association Cardio-Thoracic Surgery Guidelines myocardial recommends use only context clinical controlled studies. encouraging results obtained what expected from current studies scaffolds, might change indications considering potential benefit about natural vasomotion, neoatherosclerosis following absorption.
